Job Description
What you will do:
- Configure, build, and maintain the Epic Health Planet module to support population health initiatives.
- Collaborate with clinical and operational leaders to design workflows that support care management, preventive care, and chronic disease monitoring.
- Analyze population health data and metrics to support the design and implementation of targeted care strategies.
- Develop dashboards and reports to support performance improvement and monitor key quality indicators.
- Assist in upgrades, testing, and change management for Health Planet and related modules.
- Maintain up-to-date knowledge of Epic upgrades, functionality, and best practices in population health.
Preferred qualifications:
- Epic certification in Healthy Planet Population Health
- Experience in a hospital and/or clinical setting
