Registered Nurse (RN) – Home Health | Full-Time, On-Site

Starting Salary: $99,840/year
Our healthcare organization partner, a rapidly growing home health organization in Indianapolis is seeking a dedicated and experienced Registered Nurse (RN) to join its field-based clinical team. This full-time, on-site position offers an opportunity to make a meaningful impact in patients’ lives by delivering skilled nursing care directly in the comfort of their homes.
The Home Health RN will be responsible for managing a caseload of patients requiring clinical oversight and nursing intervention within their homes. This role involves comprehensive assessment, care planning, and coordination with interdisciplinary teams to ensure optimal patient outcomes in alignment with the organization’s high standards for quality and compassion in care.
Conduct in-home nursing assessments and develop individualized patient care plans
Administer medications, treatments, and injections as prescribed
Monitor patient conditions and report changes to physicians and other care team members
Educate patients and caregivers on disease processes, medications, and ongoing care needs
Maintain timely and accurate documentation in accordance with federal, state, and agency standards
Collaborate with interdisciplinary care team to support continuity of care
Ensure compliance with all applicable nursing regulations and safety protocols
Field-based role involving frequent travel within the Indianapolis area
Requires ability to lift up to 30 pounds, bend, kneel, and stand for extended periods
Must be comfortable entering patients’ private residences and adapting to diverse home environments
Full-time, 40 hours per week
Day shift; occasional flexibility based on patient needs
Starting salary: $99,840/year
Mileage reimbursement available
Active RN license in the state of Indiana
Minimum of one (1) year of recent Home Health nursing experience required
Current CPR certification
Valid driver’s license, reliable transportation, and auto insurance
Proficient in electronic health records and clinical documentation
Strong clinical judgment, organizational skills, and interpersonal communication
